A Letter from the AuthorsDear Readers, As a former medical student, I vividly remember the smart and efficient house staff who were managing large services of 20 or more very sick patients, each one's critical facts condensed onto a small index card. When presenting a case, attendings would always tune out during all the extraneous history and normal findings until the student finally got to the critical "buzzword" information for that disease. Today the amount of known medical information is increasing exponentially - but the time available to study, and the human capacity to learn, remains constant. This challenges all educators to create increasingly effective ways to present and package critical learning. Blackwell's Underground Clinical Vignettes series is our ongoing attempt to make case-based learning fun, efficient and accessible to medical students and other healthcare professionals. Case-based books have been around a long time, but our "supra-prototypical" format is a bit different. Instead of trying to be "realistic" and present an unknown case, with lots of irrelevant or normal findings, we focus only on the "pertinent positives and negatives." This is exactly the way busy clinicians think about patients and present cases to each other. Distilling complex and sometimes contradictory medical information into a capsule summary (be it for a 3x5 card, a 2-minute bullet presentation, or for a referral letter to a colleague) is a critical skill for medical professionals. In developing these books we managed a database of nearly 1000 diseases. Each disease could be the topic of an entire book, and distilling the content down to a 1-2 page supra-prototypical scenario has proved both challenging and gratifying. We hope that using these books will help students master this skill early in their careers. In addition, we would like to offer our thanks to the many people involved in this project from its conception. We received assistance from an extensive international team of student contributors and faculty reviewers. The team includes young medical educators from the US, UK, Mexico, India, Pakistan and Syria. Most of our team works via e-mail, with occasional meetings in California, New York and Connecticut. Our publisher, Blackwell Science, has assisted us in many ways, including providing bright new designs, contributing many images for the two new color photo atlases, and a lot more. We hope you enjoy the new series and spread the word! Please help make the next editions even better by contributing your comments, corrections and suggestions. We were very pleased with the overwhelmingly positive student feedback for the 2nd edition of our Underground Clinical Vignettes series. Well over 100,000 copies of the UCV books are in print and have been used by students all over the world.
Over the last two years we have accumulated and incorporated over a thousand "updates" and improvements suggested by you, our readers, including:
- many additions of specific boards and wards testable content
- deletions of redundant and overlapping cases
- reordering and reorganization of all cases in both series
- a new master index by case name in each Atlas
- correction of a few factual errors
- diagnosis and treatment updates
- adition of 5-20 new cases in every book ...
